On the day of your surgery you will be asked to fill out a few forms. These notify us about any allergies you might have, any medical conditions, and any other information that may help us serve you better.
On the day of your surgery you might be a bit anxious, and you may not have the desire to fill out these questionnaires. We suggest you complete the forms below to help speed up your check-in process. You can fax them, e-mail them, or bring them on the day of your surgery.
A few minutes at home is worth more than any amount of paperwork at the door.
Download and complete the forms below. Please write clearly, and do not leave the medication and allergy sections blank — those are the ones your nurse and anesthesiologist read first.
Once your forms are complete, send them to us in whichever way is easiest for you:
Verification of insurance coverage and payment arrangements must be taken care of a week before your scheduled surgery, unless otherwise discussed with the center's patient accounts administrator. See insurance and payment.
An advance directive lets you state, in writing and ahead of time, what kind of care you would want if you were unable to speak for yourself. You are not required to have one in order to be treated here, and having one does not change the care you receive.
The packet above includes a copy of the Patient's Rights along with the advance directive forms. Please read it, and bring any questions to your nurse on the day of your surgery.

Questions about your procedure itself — medications, diet, what to stop taking and when — are best answered by the surgeon with whom your surgery is scheduled.
